TheatreKids Waiver & Release Form. By registering my child, I acknowledge that TheatreKids activities, including dance, acting, singing, rehearsals and performances, involve inherent risks. I understand that TheatreKids and its instructors will take reasonable measures to provide a safe, supportive environment. I authorize emergency medical assistance for my child if needed and agree to provide accurate medical and relevant information. I also consent to photographs and videos of my child being used for TheatreKids promotional purposes, including its website, social media and advertising.
